To register for the Patient Access service, please contact the surgery to obtain the required codes:

• At reception or
• By phone on 01902 600833 or
• Email walccg.lockstownpractice@nhs.net but only if you have previously provided the Practice with your email address. Please include your full name, date of birth and address in the email.

If you have an NHS App log-in you can use this and do not need to register.
