If you wish to register click on the link below to open the form. When you have completed all of the details, email us the form and your ID papers or hand in to the reception desk when you visit the surgery.
You can also collect this form from our reception desk.
Note that by sending the form you will be transmitting information about your self across the Internet and although every effort is made to keep this information secure, no guarantee can be offered in this respect.
The area covered by the practice includes:
New Invention, Short Heath, Ashmore Lake, Spring Bank, Portobello, Little London, Bentley, Lane Head, and County Bridge.
We cover WV12 & WV13, parts of WV1,WS2, WV11, WV14, WS10, we do not cover WV2, WV3, WV4, WS1

Please ask at reception if we cover your address or the address you are moving to, or see our map in the waiting room for our catchment area. We will not accept patients out of our area.
Since January 2015, it has been possible to register some patients that live out of our catchment areas.

If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.
You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.
To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.
